Cost-effective Osteopathic Manipulative Medicine: A Literature Review of Cost-effectiveness Analyses for Osteopathic Manipulative Treatment
Russell Gamber, Shane Holland, David P. Russo, des Anges Cruser, and Peter E. Hilsenrath
J Am Osteopath Assoc 2005 105: 357-367. [Abstract] [Full Text] [PDF]

Despite the value osteopathic manipulative medicine may offer to healthcare consumers in a managed care, evidence-based healthcare system, very little research has been published on the cost-effectiveness of osteopathic manipulative treatment compared with other treatment modalities. The authors conducted a literature review of articles published between January 1966 and June 2002 and evaluated the purpose, methods, findings, and conclusions of each study for how the cost-effectiveness of osteopathic manipulative medicine was studied. The authors conclude that the osteopathic medical profession needs to conduct and publish research that is consistent with current practices in conventional medical literature.

MEDICAL EDUCATION:

Do Women Prefer Care From Female or Male Obstetrician-Gynecologists? A Study of Patient Gender Preference
Amy M. Johnson, Peter F. Schnatz, Anita M. Kelsey, and Christine M. Ohannessian
J Am Osteopath Assoc 2005 105: 369-379. [Abstract] [Full Text] [PDF]

A self-administered survey was designed for and distributed to patients in obstetrics and gynecology waiting rooms in Connecticut to determine whether there were any patient preferences with regard to the gender of physicians providing obstetric and gynecologic care within this population. The majority of women surveyed did not select their obstetrician-gynecologists based solely on physician gender. Although a small percentage of survey respondents did indicate a gender preference, that preference rarely influenced physician selection and was only a minor consideration when compared with other desirable physician attributes. The authors conclude that men should be encouraged to enter the medical specialty of obstetrics and gynecology to foster an optimal demographic balance in this specialty field--an equal distribution of men and women.

CASE REPORTS:

Benzocaine-Induced Methemoglobinemia During Transesophageal Echocardiography
Sandra Kaye Birchem
J Am Osteopath Assoc 2005 105: 381-384. [Abstract] [Full Text] [PDF]

Acquired or toxic methemoglobinemia is an uncommon complication of topically administered anesthetic agents in patients of all ages--but particularly in pediatric and elderly patients. This report describes a case of acquired methemoglobinemia that occurred after benzocaine spray was applied orally to a 69-year-old white woman weighing 175 lbwho was undergoing transesophageal echocardiography. Patient care was managed successfully. Fundamental concepts regarding methemoglobinemia are also reviewed to heighten physician awareness of this potentially life-threatening complication associated with the application of common topical anesthetic agents.
