parentheticaldescriptionof continuing medical education (CME) on the

incorrectand shouldhave appeared as "not real-time
interactive simultaneousconferencing."
In Table 5 on page 95, the total number ofCME credit hours
reportedfor the 2001 to 2003 CME cycle shouldhave been reported as13.1
instead of 13.0.
